Please rank your order of preference, listing only the classes that you would accept. We try to honor your first choice. However, the Preschool reserves the right to create optimal learning communities in each class and create balanced classrooms (i.e. age and gender).

2024/25 Application Agreement
- I understand that the $60 application fee is non-refundable and must accompany my child’s completed application.
- I understand that if enrolled, I am responsible for the payment of contracted fees and that the first tuition payment will be due on August 1.
- I understand that tuition is non-refundable.
- I understand that per Wisconsin State licensing regulation, my child must be bathroom proficient and independent to attend the Preschool. By “bathroom proficient and independent” we mean that your child must wear underpants (not pull-ups) and be able to urinate and have their bowel movement on the toilet as well as wipe themselves after.
- I understand that I will be notified of my child’s enrollment status by the beginning of February, and that if a place has not been secured, my child will be placed on a waitlist.
- I understand that if offered enrollment I must pay the $250 deposit by February to secure my child's enrollment.
- I understand that, should my child be accepted, a link to our online enrollment forms will be emailed to me in May.
- I agree to complete and submit the required enrollment forms, immunization form, and health history form by June 30, 2024.
