Summer Camp at Schlitz Audubon

Submit this secure, online form for each child, prior to submitting your family’s Camp Registration form. This profile includes a health history and emergency care plan, as well as childcare enrollment information. An automated email reply will be sent after each profile has been received.
Camper Profiles are not required to be submitted for children enrolling in Hand-in-Hand week-long camps or for children who have a Student Profile on file from the 2021-2022 school year, required for Field Days, Homeschool, Wild Wednesdays, and Nature Mondays program participation.
This year, due to licensing, immunization forms are required for only preschool (K4 & K5) campers. These forms are not required for initial registration, and can be mailed, dropped off, or faxed directly to the Center (414-352-6091) from the child’s doctor’s office and must be submitted prior to their first day of camp. Parents may also fill out this form, especially if they have chosen to opt-out of certain immunizations.
Important Camp Notes
An automated email reply will be sent after submitting each Camper Profile, as well as after submitting the Camp Registration form.
Families will receive camp confirmations and invoices within 2 business days of submitting the Camp Registration form.
We allow a maximum of six camps for the entire summer, per camper.
Cancellation & Change Policy
To cancel or change camps, please email our registrar at email@example.com.
A refund of registration fees, minus a $25 non-refundable deposit, will be given for any camps canceled 14-days or more prior to the first date of camp. There will be no refunds for camps canceled with less than 14-days notice. The cancellation policy is not dependent on the status of camp waitlists.
If we receive instruction from the CDC, North Shore Health Department, or other camp-related officials that camp programming should be discontinued, we will offer camp families a pro-rated refund.
At this time, lunch supervision will not be available for the Summer Camp 2021 season. If a camper is enrolled in both a morning and afternoon camp session within the same week, they must be picked up in between camp sessions.
If your camper may require medication while at camp, please complete a Medication Information and Authorization Form prior to your child’s first day of camp.
